Presidential Medallion Form

To qualify for the Presidential Medallion a student must earn all five coins from the Presidential Coin series. This form must be completed with all appropriate signatures and submitted to Dean of Students’ office to earn the medallion. The President’s office will coordinate a time and place for you to receive the medallion once you have received all five coins. This would typically be at Graduation.
